Effective date:1. Introduction
Welcome to the Employee Wellness Hub ("EWH"), a digital platform operated by LifeX ("LifeX," "we," "our," or "us"). The EWH is provided to LifeX Research Associates ("you," or "users") to view benefits, medical claims, and related health information. By accessing or using the Employee Wellness Hub, you agree to comply with and be bound by these Terms of Service ("Terms"). If you do not agree with these Terms, please do not access or use the EWH.
2. Eligibility and access
Access to the EWH is limited to current LifeX Research Associates and authorized users. You are responsible for maintaining the confidentiality of your login credentials and for all activity conducted under your account.
Unauthorized access, sharing of login information, or attempts to use another user’s account are strictly prohibited.
3. Authorized use
The Employee Wellness Hub is intended solely for:
Viewing your own health benefits, claims, and coverage information.
Managing and reviewing your LifeX-provided benefits.
Interacting with LifeX’s internal systems in connection with employee wellness and insurance programs.
Users agree not to:
Access or attempt to access information belonging to another person.
Modify, copy, reproduce, or distribute any part of the EWH platform.
Reverse engineer or interfere with the platform’s code, security, or functionality.
Use the platform for any purpose that violates applicable laws or LifeX policies.
4. Data privacy and HIPAA compliance
All information accessed through the EWH is protected under HIPAA and LifeX’s Privacy Policy. By using the platform, you acknowledge that your information will be collected, stored, and processed in accordance with LifeX’s Privacy Policy.
LifeX takes appropriate administrative, technical, and physical safeguards to protect your data. You are responsible for ensuring that your access device and credentials remain secure.
5. System availability and maintenance
LifeX strives to maintain continuous access to the Employee Wellness Hub but does not guarantee uninterrupted service. The system may experience downtime for maintenance, updates, or unforeseen technical issues. LifeX is not liable for any damages arising from temporary service interruptions.
6. Intellectual property
All content, software, and materials within the Employee Wellness Hub are the intellectual property of LifeX or its affiliates and are protected by applicable copyright and intellectual property laws. You may use the platform only as authorized under these Terms.
You may not copy, distribute, or use any content or technology from the EWH for commercial purposes or any purpose not explicitly authorized by LifeX.
7. User responsibilities
By using the EWH, you agree to:
Provide accurate and current information.
Use the platform solely for lawful and work-related purposes.
Immediately notify LifeX of any unauthorized access or security concerns.
Any misuse of the system may result in disciplinary action under LifeX’s employment policies and/or termination of access.
8. Limitation of liability
The Employee Wellness Hub is provided "as is" and "as available" without warranties of any kind, express or implied. LifeX is not responsible for:
Errors or inaccuracies in displayed information.
Loss of data resulting from user negligence or technical issues.
Any indirect, incidental, or consequential damages arising from your use or inability to use the platform.
9. Termination of access
LifeX reserves the right to suspend or terminate your access to the EWH at any time, with or without notice, for violation of these Terms or for any operational reason determined by LifeX.
Upon termination, you must immediately cease all use of the Employee Wellness Hub and destroy any materials or data obtained through it, unless retention is required by law or internal policy.
10. Changes to these terms
LifeX may modify these Terms of Service at any time. Updated Terms will be made available within the Employee Wellness Hub and include a new Effective Date. Continued use of the platform after updates constitutes your acceptance of the revised Terms.
